Previous School Attended

Birth Certificate

Current Proof of Residency

Updated AL Shot Record

Custody/Parental Right Papers if applicable

Gaylesville will host Open House Thursday August 4th from 4:00 to 6:30pm

Receive Schedules

Receive Lockers 10 dollar donation

Receive Parking Tags 20 dollars must have Drivers License and Proof of Insurance
